If you were born in Walsall and you need a copy of your birth certificate you should apply to Walsall Register Office. Applications can be made using our online ordering service, in person, by letter or by completing our application for a birth certificate form and sending it to us in the post.
If the birth did not take place in the district of Walsall, you will need to contact the register office in the area in which the birth occurred for a certificate.
You will need to supply the following information:
- full name at birth
- date and place of birth
- full names of both parents
- mother’s maiden surname
- reason for application
